About the service
The Hospital Discharge Team provide discharge planning and assessment for patients requiring a pathway supported discharge including Fast Track.
We ensure completion of Discharge Information Sheet and quality assure all sheets completed from the wards.
Co-ordination of the Hospitals Single Point of Access ensuring all of Discharge Information Sheets are distributed to the correct CCG locality.
All queries and escalations relating to delayed discharges are to be directed to the team.
