- Transition clinics
As this is a paediatric service young people are discharged from the service when they reach 18 unless they fall under SEND guidance. In your last appointment before your 18th birthday your clinician will establish the type of care you need, and which service can best provide this for you. You will then be transferred to the appropriate adult service to continue your treatment, this may be your GP or the adult ADHD service.
